We thank you for your interest in our services. If you are interested in receiving our services, please submit the
Intake Form to our office to begin the intake process. It is our desire to provide the most appropriate services to
your child/teen and the following information will help us in doing so.
We can provide various types of services based on each student’s need, including a parent consultation visit,
evaluation, individual weekly therapy sessions, small group therapy, and/or professional training. Our lengthiest
wait list is for after-school weekly individual therapy appointments, as there are a limited number of these
appointments available after school. We must wait for clients to exit therapy before we can offer an appointment
during that time to other clients. While many students receive weekly therapy for 2-6 months, we cannot
guarantee how long it will take before an appointment can become available for you.
Requests for being placed on the wait list may take up to a week to process, but you will be placed on the list from
the time your request is submitted. We ask that you please be patient & do not submit multiple requests, as this
will only delay our efforts to make appropriate contact with all of those waiting.
If you have completed the new patient form, or communicated with someone in our office about placing your name
on the waiting list, then we will be contacting you just as soon as an appointment becomes available. We
appreciate your patience, as we understand that the wait time for initial consultations may be longer than you
anticipated. We look forward to meeting you and helping you, your child or teen. If you wish to speak with
